· Schedule: Monday, Tuesday, and Thursday 8a-4:30p and Wednesday 10:30a-7p
· Patients per day: 18
· Practice Setting: Wellness Clinic
· Scope: Inpatient
· Licensure: Michigan state medical license, Basic Life Support (BLS), Board Certified and Drug Enforcement Administration (DEA)
· Credentialing Timeframe: one week
