Job Description
Position Summary
We are seeking a caring, motivated, and organized SCN Screening & Navigation Specialist to help individuals access the support and services they need. As one of the first people members interact with, you will play a key role in understanding their needs, determining program eligibility, and guiding them toward resources that can improve their health, stability, and overall well-being.
ResponsibilitiesMember Screening & AssessmentConduct screenings to evaluate eligibility for SCN programs and services.
Collect and review demographic, health, insurance, and social needs information.
Identify challenges that may impact a member's health and quality of life, including housing instability, food access, transportation barriers, unemployment, behavioral health concerns, and other social determinants of health.
Guide eligible individuals through the enrollment process.
Connect members with available services and community resources.
Facilitate access to transportation assistance, nutrition programs, personal support services, health education, and other supportive benefits.
Ensure members understand their available options and next steps.
Maintain complete and accurate member records.
Ensure all documentation complies with SCN and Department of Health guidelines.
Monitor outstanding assessments and follow up to obtain missing information.
Support the development of personalized service plans.
Partner with care managers, healthcare providers, behavioral health programs, hospitals, and community organizations to coordinate services.
Monitor member participation and service utilization.
Conduct outreach and provide support when members encounter barriers or require additional assistance.
High School Diploma or equivalent required.
Strong communication, listening, and relationship-building skills.
Ability to conduct assessments and gather information with accuracy and sensitivity.
Excellent organizational and time-management abilities.
Self-motivated and able to work independently.
Bilingual in English and Spanish or English and Yiddish required.
Experience in healthcare, social services, case management, community outreach, or a related field is a plus.
